At the request of GPs, Health Visitors, Hospitals, Social Services, the Hospice, and the District Nursing Team visits clients who are house bound or unable to attend the Treatment Room at Cleveleys Health Centre.

The team assesses the nursing needs of clients in their own homes and these are some of the areas of care we are responsible for providing:-

Care of the terminally ill.
The Management of clients with epidurals, syringe drivers and Hickman lines.
Post-operative care following discharge from hospital including the management of dressings, wound care and the removal of sutures and clips.
Pressure care and advice.
The management of patients with leg ulcers.
Taking blood samples.

As part of the service provided to all our patients with Diabetes the District Nursing Team offers advice and educational support to those clients who are unable to attend the Diabetic Clinic. This includes teaching clients how to administer their own insulin.

Administering prescribed medication by injection.
Ear syringing if clients are unable to attend the Ear Clinic held in the Treatment Room.
Teaching the administering of eye drops if client unable to do their own.
Influenza Vaccinations either as part of a clinic held at the Crescent Surgery or in the clients home.
All clients over the age of 75 will be invited to attend the surgery for a health assessment, which may be conducted by a member of the District NursingTeam. If clients are unable to attend the surgey then a visit to the clients home can be arranged.
Incontinence assessment including catheter care, bowel care and advice.
Health promotion support and advice.
Teaching and supervision of all aspects of care to clients and their relatives/carers.
Health needs assessment at the request of the GP or Social Services in clients own home, rest home and nursing home.
Referrals to Social Services, Chiropody and Occupational Therapy or other relevant health professionals.
Bereavement Counselling.

The health visitors are closely involved in the health care of patients, particulary children, expectant mothers and children with special needs. The team may be contacted at the health centre on 01253 864122.

The health visitors run a baby clinic on Monday mornings between 9.30 and 11.30am. A doctor is in attendance at these clinics twice a month. A children’s immunisation clinic is held on Tuesday from 9.30am to 12.15pm

Counsellors

The Mental Health Helpline can be the first port of call for anyone who is concerned about their own mental health or that of a friend or relative. It is a FREEPHONE, CONFIDENTIAL INFORMATION and LISTENING service and the number will not appear on the telephone bill.
